Care Coordinator – Supported Living Services
Location: Southampton & surrounding areas
Salary: £28,000 – £29,000 per annum (DOE)
Hours: 40 hours per week
Holidays: 25 days + bank holidays
On Call: Participation in the on call roster – additional remuneration
As part of the Natali Group, our supported living services in the Southampton area are seeking an experienced, organised, and proactive Care Coordinator to join the branch team. In the current steady state, the service delivers approximately 1500 care hours per week & 50 carers, with scope to scale and grow at pace.
This is a critical operational role within the service, acting as the right hand to the Registered Manager and supporting the smooth, compliant, and high-quality day-to-day running of operations. You will work closely with support staff across multiple sites, ensuring individuals receive person-centred, consistent care that enables them to live independently as the service develops.
About the Role
As the Care Coordinator, you will play a central part in the operational success of our supported living services, including:
- Care Coordination & Rota Management
- Creating and maintaining accurate weekly rotas, ensuring all packages are safely and effectively covered.
- Matching Support Workers to individuals based on skill, suitability, and location.
- Managing staff availability, annual leave, sickness cover, and efficient resource allocation (mileage, time, capacity).
- Supporting the integration of new service users into the rota system and briefing staff on individual needs and support plans.
Compliance & Quality Assurance
- Supporting the Registered Manager in ensuring ongoing CQC compliance across supported living settings.
- Monitoring service quality, identifying improvements, and taking action to resolve issues.
- Ensuring all client files, documentation, and electronic care records are accurate, up to date, and audit-ready.
- Assisting with safeguarding alerts, incident follow-up, and compliance audits.
Workforce Support & Leadership
- Working closely with the Registered Manager to maintain a motivated, positive, values-driven team culture.
- Participating in staff supervisions, one-to-ones, appraisals, and staff development planning.
- Supporting recruitment, onboarding, and induction of new team members.
- Acting as the deputy for the Registered Manager during periods of absence, ensuring continuity of service.
Operational Support
- Building strong relationships with local authority teams, families, and external professionals.
- Supporting service improvements and helping embed new processes following the acquisition.
- Covering on the on-call rota for out-of-hours emergencies (paid).
- Being willing to step into frontline support on rare occasions where staffing levels require it.
Experience & Knowledge
- Previous experience as a Care Coordinator or Senior Support Worker in supported living or domiciliary care.
- Strong understanding of rota building, staff deployment, and service continuity.
- Knowledge of CQC requirements, safeguarding, Health & Safety, and quality standards.
- Proven experience supporting individuals with learning disabilities, autism, and complex needs.

- Proactive, solution-focused, and able to work both independently and as part of a close-knit team.
- Confident IT user, comfortable with digital scheduling and care management systems.
Qualifications
- NVQ Level 3 in Health & Social Care (or willingness to work towards it).
- Enhanced DBS and full driving licence with access to a vehicle.
